Acknowledgment of Risk:

I, the undersigned participant, acknowledge that participating in fitness classes, personal training, and other physical activities offered at Epoch (the "Studio") involves inherent risks. These risks may include, but are not limited to, injuries resulting from:

1. Strenuous physical activity and overexertion.

2. Improper use of equipment.

3. Accidents occurring in or around the Studio premises.

4. Existing medical conditions or physical limitations.

I voluntarily assume all risks associated with participating in these activities.

Medical Clearance:

I confirm that I am physically fit and have no medical conditions that would prevent my participation in fitness activities. I understand it is my responsibility to consult a physician before starting any exercise program.

Release of Liability:

In consideration of being allowed to participate in the activities provided by the Studio, I hereby release, waive, discharge, and hold harmless the Studio, its owners, instructors, employees, agents, and affiliates from any and all claims, liabilities, damages, or expenses, including those caused by negligence, arising out of or in connection with my participation in Studio activities or use of Studio facilities.

Indemnification:

I agree to indemnify and hold harmless the Studio from any claims, actions, or damages resulting from my participation, including but not limited to legal fees incurred in defending such claims.

Photo and Media Release:

I grant permission for the Studio to take and use photographs or videos of me for promotional purposes, including but not limited to social media, website, and marketing materials. I understand that I will not be compensated for the use of these materials. 

Studio Policies:

I agree to adhere to the Studio's rules and policies, including proper use of equipment and respectful conduct towards staff and other participants. I understand that failure to comply may result in suspension or termination of my access to the Studio without refund.

Binding Agreement:

I understand that this waiver is binding upon me, my heirs, executors, administrators, and assigns. I have read and fully understand this waiver and release of liability and sign it voluntarily.
